Description:
32 hour nursing position working directly with Resident Care Director. Wellness nurse participates in health service delivery, provides direct care service and supervises person care. Assists in the health Promotion and Care program for residents.
Responsibilities:
• Oversees and/ or provides health services within assisted living regulations that apply.
• Develops and maintains health promotion and care program for specified Assisted Living area.
• Coordinates efforts of multi-disciplinary team
• Assures that service plans are established for all assisted living residents and maintains records.
• Works with Director of Nursing, Resident Care Director, Social Services, and other departments to provide follow up care and family conferences.
• Informs administration of concerns of residents, families, and staff when appropriate
• May administer medications
Requirements:
• Desires geriatric nursing position
• RN/ LPN with two or more years of supervisory/ management, clinical experience
• Ability to communicate verbally and in writing with residents, families, physicians, outside agencies, and all levels of staff.
• Applies good judgement and resourcefulness in emergency situations
